Telehealth Consent

You are consenting to receive medical care through telehealth when you request prescription treatment through Liquivida Wellness Center Aventura. This page explains who treats you, what they decide, and what telehealth cannot do. Submitting an intake form or requesting a telehealth consultation counts as agreement to the terms below.

What Telehealth Is

Telehealth means your medical care happens over electronic channels: an online intake form, secure messages, a phone call, or a video visit. A licensed medical provider reviews the health information you submit and decides whether to prescribe treatment, without an in-person appointment.

Who Provides Your Care

Licensed medical providers with Elite Care Health, an independent medical provider group, make all clinical decisions. Rx Ave Health, a licensed pharmacy, fills and ships prescriptions. Liquivida Wellness Center Aventura does not issue or fill prescriptions through this website.

No Guarantee of Treatment

Completing an intake form or selecting a program does not guarantee treatment. A provider prescribes only what they judge clinically appropriate for you, and they can first ask for more information, lab work, or a consultation. They can also decline to prescribe.

Risks of Telehealth

A telehealth evaluation depends on the information you submit and on working technology. Connections fail, responses take time, and the provider can decide the information in front of them is not enough. In that case you will be asked to complete an in-person evaluation before treatment.

Your Responsibilities

Give accurate and complete answers about your health history, current medications, and allergies. Follow the dosing instructions your provider sets, and report side effects to your provider as soon as they appear.

Emergencies

Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.

Privacy

Your health information is protected under HIPAA. Our Privacy Policy and HIPAA Notice of Privacy Practices describe how it is collected, used, and stored.

Availability

Telehealth services may be available in all 50 states where clinically appropriate and permitted by applicable law.

Withdrawing Consent

You can withdraw this consent at any time by contacting us. Withdrawal does not undo care already provided.
